Timber Brown StazOn ink pad (Tuskineko)

Notepads, 3″x5″ (Mead)
Circle Punch (Stampin’ Up!)
Glue (Aleene’s Tacky Glue)
Directions:

Cut a piece of cardstock 3.25″ x 10.25″ and score at 5″ and 5.25″.Fold on score marks.
Cut two rectangle piece measuring 1.5″x2.25″ out of patterned paper and one piece out of solid paper.
